How to restore a lost or damaged ID card of a participant in hostilities: instructions

Such an opportunity is available to those defenders who received the status of UBD according to the decision of the interdepartmental commission of the Ministry of Veterans Affairs of Ukraine.

The press service of the Lviv OTCC and SP told how to restore a lost / damaged certificate of a participant in hostilities. First, you need to collect the necessary documents:

Application for replacement of a damaged / lost certificate specifying the reasons for the loss. Be sure to include a mobile phone number in the application.
In case of loss of the certificate (if possible), an announcement in the newspaper about the loss of the document with the name, surname, patronymic indicated on it.
If the certificate is damaged, it is a damaged certificate.
A copy of the lost certificate (if available) or the number, series of the certificate and the date of issue.
Copies of passport pages (1, 2 and 11, 12), copies of the identification number assignment certificate.
3*4 size photo cards – 2 pcs.

Next, you need to send the documents to the e-mail address of the Ministry of Veterans: dpi@mva.gov.ua (photo cards – by postal service of JSC “Ukrposhta” to the address of the Ministry of Veterans, Kyiv, 12 Muzeyny Ave., 01001).
