There are no forms of ID cards for recipients of assistance in Lviv

There are no forms for state-provided ID cards, so for now residents will be issued a certificate

District departments of social protection in Lviv receive many requests for certificates of assistance to people with childhood disabilities and children with disabilities, as well as people who are not eligible for a pension. However, there are currently no forms of these certificates provided by the state. So temporarily, residents will be issued a certificate of receipt of assistance.

“These are strictly accountable forms, and their circulation is strictly controlled. The forms are made on the basis of the order of the Ministry of Social Policy of Ukraine and are received centrally through the departments of social protection of the population of regional state administrations, in accordance with the Order of the Ministry of Social Policy of 11.01.2019 No. 35. To resolve this situation, we have repeatedly sent appeals regarding the need to provide forms of certificates, but they are currently not available,” the social protection department of the Lviv City Council explains.

Therefore, given the current situation and the need to provide people with disabilities, including children with disabilities, or their legal representatives with a document that would certify the fact of state aid, the city’s social protection departments will issue a Certificate of Receipt of Assistance, approved by the Order of the Ministry of Social Policy of 19.09.2006 No. 345 (as amended by the Order of 18.10.2023 No. 398-N). However, it is important to understand that this certificate is valid if you have supporting identity documents. However, it does not replace a certificate.
