
Service personnel who are residents of Lviv region can make use of a free social rehabilitation (habilitation) service. Accommodation, meals and the services themselves are fully funded by the state under the current regulations.
The programme provides for several facilities where defenders can undergo a recovery course.
A full recovery course lasting 21 days is offered by the Prykordonnyk – Nemyriv sanatorium (21 Kurortna St, Nemyriv urban-type settlement, Yavoriv district, Lviv region). The facility specialises in rehabilitation of the musculoskeletal system, nervous system and skin.
An optimal course lasting from 12 to 16 days is available at the following institutions:
- enterprise “Zakhidnyi Rehabilitation and Sports Centre” of the National Committee of Sports for the Disabled of Ukraine (1 Rivnykh Prav ta Mozhlyvostei St, Yavoriv village, Sambir district, Lviv region) — physical and sports rehabilitation, adaptation;
- state enterprise “Sanatorium “Kryshtalevyi Palats” of the State Management of Affairs (35 Sukhovolia St, Truskavets, Lviv region) — treatment of the gastrointestinal tract and general strengthening of the body;
- LLC “Hotel and Resort Complex “Karpaty”” (2 Karpatska St, Truskavets, Lviv region) — modern diagnostics, balneological procedures and hydrotherapy.
To receive the rehabilitation (habilitation) service, the serviceperson submits a set of documents to the chosen service provider.
The following must be prepared:
- an application in free form;
- a copy of the passport of a citizen of Ukraine (booklet or ID card together with an extract from the register of the territorial community; if the documents are submitted by a legal representative, copies of the documents of the person on whose behalf the application is submitted and a document confirming the representative’s authority must be attached);
- a copy of the certificate on assignment of the taxpayer’s registration number or a notice of refusal to accept it on religious grounds;
- a copy of the standard-format war veteran’s certificate (in accordance with Resolution of the Cabinet of Ministers of Ukraine of 12.05.1994 No. 302 as amended by Resolution of 22.08.2018 No. 632) or, if it is not available, a copy of one of the certificates confirming participation in measures to ensure national security and defence, repulsion and deterrence of the armed aggression of the Russian Federation, issued in the forms approved by Resolution of the Cabinet of Ministers of Ukraine of 20.08.2014 No. 413;
- a certificate from a medical institution indicating the diagnosis confirming the need for rehabilitation (habilitation);
- a referral for receiving the social rehabilitation (habilitation) service, completed by a doctor;
- copies of documents confirming family ties (marriage certificate, child’s birth certificate) — if required;
- the original certificate confirming student status — for children aged 18 and over who are studying full-time at a general secondary, vocational or higher education institution;
- consent to the processing and use of personal data in accordance with the Law of Ukraine “On Personal Data Protection”.
Other documents may also be submitted at the person’s discretion, if needed in view of the specific situation.
The provider concludes a contract for the provision of rehabilitation (habilitation) services with each recipient. If one of the serviceperson’s family members is staying at the facility together with them, they may also, at the recipient’s choice, be provided with accommodation, meals and socio-psychological support services.
