There is a rehabilitation aids rental point at the St. Panteleimon Regional Center for Comprehensive Rehabilitation for Children and Persons with Disabilities. There you can rent various types of equipment: from crutches to verticalizers. The full list of equipment is available here.
“This service is free of charge. It can be used by people who need special technical rehabilitation equipment. These are children with disabilities under the age of 18, people with disabilities, as well as other specific categories of the population – the elderly, combatants, and victims. It is available to residents of Lviv region and internally displaced Ukrainians who are registered and reside here.
I would also like to emphasize that the center has a fairly wide range of equipment. This includes a stand (verticalizer) with elements of fixing the torso; orthopedic children’s chairs; walkers on wheels; active wheelchairs, including children’s chairs; wheelchairs with electric drive; bathroom accessories – chairs, seats, benches, steps for the bathroom and shower, toilet chairs and other things,” said Roman Andres, head of the social protection department of the Lviv Regional State Administration.
To receive this service, one of the parents, guardians or legal representatives of a child with a disability, a person with a disability or their legal representative must apply to the rehabilitation aids rental center, which operates on the basis of the St. Panteleimon Regional Center for Comprehensive Rehabilitation for Children and Persons with Disabilities. It is necessary to submit an application, certified copies of documents and conclude an agreement. As for the list of documents (copies thereof), you must have
- Passport of a citizen of Ukraine (1-2 pages (3-6 if there are any stamps)) or ID card or birth certificate of a child who needs to rent technical and other rehabilitation equipment;
- Passport of a citizen of Ukraine (1-2 pages (3-6 if there are any stamps)) or ID card of the legal representative of the person (if any) who needs to rent technical and other rehabilitation equipment;
- pension certificate of the person who needs to rent technical and other rehabilitation equipment (if any);
- registration number of the taxpayer’s account card of the person requiring the rental of technical and other rehabilitation equipment (if any);
- taxpayer registration number of the representative (if any);
- certificate of an internally displaced person who needs to rent technical and other rehabilitation equipment (if available);
- an individual rehabilitation program for a person with a disability (for a person with a disability and a child with a disability) or a conclusion of the Medical and Qualification Commission (for other persons, persons with a disability whose disability group was established before January 1, 2007 for an indefinite period or has not expired) containing indications for the provision of auxiliary rehabilitation means.
As for the term of the contract for the temporary use of an auxiliary rehabilitation device, it can be concluded for 6 months with a possible further extension.
