The Sokal community is conducting a survey for IDPs about their experience in receiving administrative services from the community

The Center for Joint Action (NGO) in the framework of the World Bank’s Health Sector Reform Support Project, funded by the Swiss Agency for Development and Cooperation, is launching an assessment of the level of COVID-19 vaccination among internally displaced persons hostilities in the communities of Lviv region.

If you are an internally displaced person, please fill out this form. It contains questions about your vaccination experience and interacting with social and health services from the community you moved to. Estimated time to complete the questionnaire – 10 minutes.

The survey is confidential, so we do not ask you to provide your personal information and guarantee the anonymity of these questionnaires. The information obtained will be used to study the health care delivery system and to prepare communication materials on COVID-19 vaccination.

We want to ask you about your experience of receiving services in the community and what you think about counteracting COVID-19. We want the quality of services to be at the highest level for all residents of the community, and therefore your opinion is very valuable to us. Please pass the survey by June 1, 2022. It is conducted by our partners from the Center for Joint Action among IDPs. We will take your answers into account to improve the quality of community services, the city council said.
